As a Health Referral Facilitator, you will support the delivery and day-to-day operation of our health and rehabilitation programmes, working with participants through individual and group sessions to help improve their health, physical activity and wellbeing.
You will work as part of our multidisciplinary Health Team, delivering specialist programmes relevant to your qualifications and developing your skills through training to support the wider range of programmes we provide across Belfast. The role also includes referral processing, participant support, accurate record keeping, outcome monitoring and data management.
The Health Referral Facilitator is responsible to the Healthwise Coordinator / Health Manager.
What you’ll do:
* Deliver safe, effective and person-centred health and physical activity interventions across our range of health pathways. Depending on your current qualifications this will include GP Referral, MSK, Cardiac Rehabilitation, Pulmonary Rehabilitation and Cancer Rehabilitation.
* Complete initial consultations, health assessments and regular reviews, supporting participants throughout their programme and helping them achieve meaningful health and wellbeing outcomes.
*Manage an allocated caseload, ensuring participants receive appropriate support, follow-up and progression throughout their referral journey.
*Deliver both individual and group sessions across a range of GLL facilities, adapting interventions to meet the needs and abilities of participants.
*Work collaboratively as part of a multidisciplinary Health Team, sharing knowledge and supporting colleagues across different specialist pathways.
*Undertake relevant training and gain additional specialist qualifications to broaden your skills and enable you to support the wider range of programmes delivered by the team.
*Build effective relationships with GPs, healthcare professionals, and other referral partners to support an integrated referral pathway.
*Maintain accurate and confidential participant records, including referral information, attendance, reviews, outcomes and programme completion data.
*Contribute to the monitoring, evaluation and continuous improvement of our health programmes, using participant and performance data to support effective service delivery.
*Support the smooth day-to-day running of the Health Team, including managing referrals, responding to participant queries and working flexibly to meet changing service demands.
*Work across multiple locations throughout Belfast, with flexibility to support different programmes and centres as required.
What you’ll need:
*A minimum Level 3 GP Referral qualification
*A full, current driving licence and access to a car, as the role involves city-wide delivery across multiple locations.
*A willingness and ability to undertake additional specialist training and qualifications as required to support the range of health and rehabilitation programmes delivered by the team.
*Strong communication and interpersonal skills, with the ability to work effectively with participants, healthcare professionals and colleagues.
*The ability to work independently while contributing effectively as part of a multidisciplinary team.
*A genuine commitment to supporting people to improve their health, physical activity and wellbeing.
Desirable:
*Level 4 specialist qualification(s), such as Cardiac Rehabilitation, Pulmonary Rehabilitation, Cancer Rehabilitation, MSK or other relevant specialist health and exercise qualifications.
*Experience delivering specialist health or rehabilitation programmes.
Ongoing requirement of the role:
*A commitment to continuous professional development, with an expectation to undertake relevant specialist training and gain additional qualifications as required. This will enable you to support the delivery of the wider range of health and rehabilitation programmes provided by the Health Team.

*This is a full-time permanent position within GLL, with the current Health Team service supported by external funding. While the employment is permanent, the role and service requirements may change in response to future funding arrangements. Hours of work will be negotiated with you in line with the needs of the service; flexible working may be possible. Working from home is not possible in this role.
