Overview
We are seeking a dedicated GP Exercise Referral Coordinator to join our healthcare team. This role involves developing and implementing personalised exercise programmes for patients referred by general practitioners, with the aim of improving their overall health and wellbeing. The ideal candidate will possess a strong background in nursing, public health, or related fields, and demonstrate excellent communication skills to motivate and educate patients. Duties
- Assess referred patients’ health status and suitability for tailored exercise programmes
- Develop personalised exercise plans in collaboration with healthcare professionals, considering medical history and individual needs
- Deliver engaging educational sessions on topics such as nutrition, physical activity benefits, and lifestyle modifications
- Monitor patient progress through regular follow-ups and adjust programmes accordingly
- Provide guidance on safe exercise techniques and injury prevention
- Maintain accurate records of patient interactions, progress notes, and programme outcomes
- Collaborate with multidisciplinary teams including nurses, dietitians, and public health specialists to optimise patient care
- Promote community engagement initiatives to increase awareness of physical activity benefits
- Ensure compliance with healthcare regulations and confidentiality standards
Qualifications
- Level 3 Diploma in Exercise Referral (Essential)
- Proven experience in nursing, public health, teaching, or related fields
- Strong knowledge of medical terminology and its application in patient care
- Excellent public speaking and communication skills to effectively motivate diverse patient groups
- Familiarity with nutrition principles relevant to exercise programmes
- Ability to organise and deliver educational sessions confidently
- Prior experience in developing or managing exercise referral schemes is desirable but not essential
- A recognised qualification in fitness instruction or sports science is advantageous
- Commitment to promoting health equity and supporting behavioural change initiatives
